(Transfer Your Care to HCHS with Ease)

Switching to another doctor or provider may seem difficult, but Hancock County Health System just made it easier to become a patient at HCHS with its Care Connect program. This free service is offered to make your health care transition easy, whether you’re changing from a clinic across town or moving from across the state.

This service can help patients who want to be seen by a provider at one of HCHS’s four primary care clinics or want to be treated by a specialist at HCHS. At HCHS, our mission is to make health care as convenient and accessible as possible for our patients.

The change starts with your decision to make one call to HCHS. From that point forward, our team will walk you through the simple steps. A HCHS team will do it all from transferring your records to scheduling your first appointment.
